Documentation & Form Completions

We provide completion of forms, letters, and documentation when clinically appropriate. Some requests—such as disability forms, employment clearance, or school accommodations—may require an evaluation before completion. Patients will be advised in advance if an additional appointment is necessary.
Follow-Up Appointments
Duration: 30 minutes (extensions available as needed)
Price: $250 for 30 minutes; prorated at $500 per hour
Follow-up sessions focus on monitoring progress, reviewing lab results, adjusting treatment plans, and supporting ongoing healing. Longer visits may be scheduled if more time is needed.

Payment & Insurance
Direct-Pay Practice
Transcend Functional Psychiatry is a direct-pay practice. This means we do not contract with insurance companies, and payment is due at the time of service.
Superbills for Reimbursement
Upon request, we can provide a superbill (an itemized receipt) that you may submit to your insurance provider for potential out-of-network reimbursement. Please note: reimbursement is not guaranteed and depends on your insurance plan’s policies.
